North East Dementia Strategy - Caregivers and People Living With Dementia

North East Dementia Strategy Survey

Thank you for taking the time to complete this survey.

Building from previous engagements with caregivers and people living with dementia, the Ministry of Health and Long-Term Care has committed over 100 million over the next three years to improve the lives of people living with dementia and their families. The information collected from this survey will inform a regional strategy for North Eastern Ontario so those funds have the greatest impact.

Your participation and input are critical to ensure the strategy that goes forward is representative of the needs of individuals from the region, hearing from the diverse urban and rural settings across north eastern Ontario. We value your perspective as a person living with dementia or caregiver.
