CFPC EPANEL CONSENT FORM

HAVE YOUR SAY – JOIN THE CFPC EPANEL
 

The College of Family Physicians of Canada (CFPC) is establishing an ePanel and we want to hear from you.

By participating, your opinion will help guides us as we create policies, provide practice supports, and talk to government on your behalf.

• We promise no more than 6 ePanel polls per year
• We promise no more than 10 questions per ePanel poll
• We promise to listen to you – your opinions are highly valued!
• We promise to share our findings with you

Your participation in the CFPC’s ePanel is voluntary. If you decide to participate in the CFPC’s ePanel, you are free to skip any question you choose and you may withdraw at any time. If you decide not to participate in the ePanel or if you withdraw from participating at any time, you will not be penalized. You are also welcome to unsubscribe to the ePanel at any time. Members of the e-panel will not be financially compensated in any way.

While your responses are not anonymous, they will be kept confidential. We will link your poll results with the CFPC membership database and data will be analyzed only as part of an aggregate data set, not individually. All data will be stored in a password-protected electronic format.
