2019 MAZON Canada Additional Program Form

ADDITIONAL PROGRAM FORM

This is the optional form for requests for funding for additional programs (by an organization that has already filled out the main Grant Application Form). You must have already completed the main Grant Application.

For most applicants, the maximum grant request is $5,000 and the average grant in this category is $2,500. However, if your organization is an umbrella organization that provides resources to multiple locations (like a large food rescue organization or a school nutrition project that distributes to multiple schools) and you have a total annual budget over $1 million, you can request up to $12,000. The average grant in this larger category is $5,000.

A note on "organization" and "program":

Your "organization" is the institution that runs your program. Your "program" is a specific project run by that institution. For smaller organizations, they are likely to be the same - larger organizations may have many programs. These organizations should fill out an Additional Program Form for each extra program they would like considered. The program described on the main application will not be prioritized over Additional Programs. All programs will be considered equally. 

For example, here are two fictional applicants: The Waterbury Community Centre serves its low-income community with movie nights, ASL classes, art therapy classes, drop-in hours with a social worker, and an after-school homework support program that provides snacks. They are applying for funding for the snacks. For them, questions about "program" (i.e., who are the clients served, what is the program budget) should be answers   only about the after-school program, its students and its snacks. The Waterbury Food Bank, however, is a once-weekly food bank run by volunteers out of a church basement; it does not have specific sub-programs. For the Waterbury Food Bank, questions about program and organization should be treated the same.

If you are applying for multiple programs: make a funding request only for the specific program named on each form, totalling up to your maximum request. For example, if you are eligible for up to $5000 and want to request funding for three different programs, you might fill out the main Grant Application requesting $1300 for one program, then this Additional Program form twice, requesting $1700 for another program and then $2000 for the last.
