1. Have there been restrictions or gaps of longer than one year in your licensure, either continuously or if added up over the past five years of practice from the date of application? *This question is required.
This self-assessment does not guarantee the awarding of the CCFP. All applications are assessed to determine eligibility and are subject to review by the Board of Examiners of the College of Family Physicians of Canada who will make final rulings on all questions of eligibility for certification.
