Mid-Career Workers with Disabilities Survey

Thank you for taking the time to complete this survey. If you are a worker who is pursuing a career and who identifies as having a disability, we want to hear from you. Your answers will help other people understand disability for mid-career workers by finding out about its effects on you. This survey cannot identify you personally or your computer address. By completing this survey, you consent to give us your information. If you do not wish to participate, do not start the questionnaire and simply navigate away from the web page or close the tab/browser. If you begin the survey and do not wish to continue, simply navigate away from the web page or close the tab/browser and your data will not be stored.

Consent Statement: You are invited to complete this 15-minute online survey if you are 18 years or older.

Title: This research is called, “Supporting mid-career workers with disabilities through community building, education and career-progression resources”  

This study was approved by the University of Ontario Institute of Technology (UOIT) Research Ethics Board REB # 15803 on October 2, 2020. Dr.  Sue Coffey at UOIT in Oshawa is conducting this study along with Dr.  Charles Anyinam at Nipissing, Prof.  Leslie Graham at Durham College, Dr.  Celina da Silva at York University and Dr. Lorayne Robertson, UOIT. Purpose: The purpose of this survey is to hear your views as a worker with a disability. Your participation in this research is completely voluntary. Nothing in this survey will identify you. You are under no obligation to participate. If you decide not to participate, you can stop at any point before submitting the survey by navigating away from the web page or closing the tab/browser. However, after you submit the survey, your data will be gathered by the computer and you may not withdraw at that point.

Data Collection, Use, and Storage: None of the data for this study contains any identifying information. We use an online tool called SurveyGizmo to collect data. The use of SurveyGizmo involves uploaded information to a server located in Canada. Data on SurveyGizmo is in full compliance and security under Canadian law. The data exists completely separate from American data and European data. 

Data Security: The data will be downloaded from the SurveyGizmo server and stored on an encrypted data storage device that is under lock and key. We will combine all of the responses we receive, analyze them together, and report them in a summarized format. The responses that we collect from you or any other participants will never be reported individually. Our summarized findings will be shared through reports, presentations, and publications in peer-reviewed journals. The results will also be used to develop tools and resources to support mid-career workers with disabilities in Canada.

The data we collect will be securely stored on password-protected cloud storage at Ontario Tech University, which is the university of the Principal investigator, Dr. Sue Coffey. We will delete the files after five years.

Accessing Results: You will be able to access a report of the results on the National Educational Association of Disabled Students (NEADS) website []. 

Potential Risks: There are no risks to you for participating in this study. You may feel a sense of obligation to participate in the survey as the request is coming through a known online network, but your participation is absolutely voluntary. It is possible that some of the questions may upset you or cause discomfort. Because of this, you may choose to skip questions. You can choose not to participate at any time before submitting the survey by navigating away from the web page or closing the tab/browser. Your responses will not be saved.

Benefits: You may not benefit directly from participating in this study; however, sometimes people find being asked about their perspective and opinions intrinsically useful — this would be personal to each participant. This study is socially useful as it is important to hear your voice and contributes to the body of knowledge about workers with disabilities. Lastly, the research will inform the development of social innovations to support mid-career workers with disabilities.

Confidentiality and Anonymity: This survey does not collect any information to identify you. Your privacy will be respected. No information about your identity will be shared without your permission unless it is required by law. Confidentiality will be provided to the fullest extent possible by law, professional practice, and ethical codes of conduct. 

Accessibility: This survey is compatible with computer-assisted reading support systems and is compliant with the Accessibility for Ontarians with Disabilities Act (AODA) (Content Accessibility Guidelines (WCAG) 2.0, Level AA).
