Primary Health Care Community Advisory Committees - Call for Interest

Thank you for your interest in the Labrador-Grenfell Health Primary Health Care Community Advisory Committee (CAC). This short survey is intended to capture information for our call-for-interest to select members for the CAC. We will contact those members who are selected.

Primary Health Care
Primary health care is typically the first and most frequent point of contact for someone within the health care system. Effective primary care is oriented around health and wellbeing and not only the treatment of diseases, and acts across the life span.

Labrador-Grenfell Health Community Advisory Committee
Labrador-Grenfell Health Community Advisory Committee will meet every second month (6 times per year) in the evening on a day that is convenient for the selected members. The purpose of the LGH CACs is to engage community members, including clients and their families, in the planning, implementing and evaluating of primary health care services in the each of our Primary Health Care regions and to collaborate in improving the health status of the population. No prior experience with committees is required to participate in this committee.

By providing your contact information and responses below you are completing the requirements to apply for the CAC and you are consenting to the release of this information to Labrador-Grenfell Health and giving permission for us to contact you regarding this purpose.
