Purpose of the Survey
At Central Health, we are committed to providing quality care to patients and their families. Central Health is a Person- and Family-centered Care organization, which means working together with patients, families, health care providers, staff and volunteers to provide respectful, compassionate, quality and safe care.

In an effort to learn from our COVID-19 response, the following feedback form has been developed to gather information and identify priorities for improvement.
